To secure your appointment, a $50 non-refundable booking fee is collected at the time of scheduling. Please read the following terms carefully.
A 10% processing fee will be deducted from your $50 booking fee. The remaining balance will be refunded within 5–7 business days.
The $50 booking fee is fully forfeited. No refund will be issued.

Any balance not covered by insurance is your responsibility, due at time of service. Insurance estimates are not a guarantee of coverage.
All dental services are final upon completion. Concerns after treatment will be addressed with a complimentary follow-up evaluation — this does not constitute grounds for a refund.
